Introduction: Intestinal dysbiosis is a risk factor of obesity and its sequelae. Dysbiosis in easy-to-reach parts of the intestines is well-studied, in contrast to the less accessible parts, as the jejunum to which Roux-en-Y bypass surgery offers accessibility. In this study we assessed the jejunal microbiome of severely obese patients undergoing Roux-en-Y bypass surgery. Methods: The taxonomical composition of the jejunal microbiome was assessed in the mucosa of the excised jejunal sections by shotgun metagenomics. Correlations were assessed between the clinical records and the taxa of the microbiome. Results: A surprisingly large number of genera were identified in the jejunum among which classical intestinal, opportunistic intestinal and transient (i.e. food-associated) genera were detected. Based on the genera of the jejunal microbiome patients separated into three clusters (clusters 0-2) with differences in the clinical markers related to tissue damage/organ function. We identified multiple, although statistically weak associations between jejunal genera and the patients medical records, as metabolic (glycemic control and lipid metabolism markers), cardiovascular and tissue damage/organ function parameters. Among the correlations cluster 0 and 2 species appeared frequently, but cluster 1 species did not appear. Of the genera Limnospira, Limosilactobacillus, Phocaeicola, Alistipes, Streptomyces, Lactiplantibacillus, and Prevotella had multiple correlations with clinical markers. Conclusion: Hereby, we identified genera of the jejunal microbiome correlating with clinical markers of severe obesity.
