Archive description

A hospital outbreak cluster of diarrhoea with one death in three previously healthy healthcare workers, with inconclusive negative multiplex polymerase chain reaction (PCR) for common gastroenteritis agents, was investigated. Further next-generation sequencing and viral culture on the stool revealed rotavirus C (RVC) and human astrovirus MLB (HastV-MLB) in the deceased doctor. Subsequent PCR confirmed the presence of HastV-MLB RNA in stool from another healthcare worker involved. While the viral load of RVC was high in serum, stool, and other tissue specimens in the deceased who succumbed fromto shock and disseminated intravascular coagulation, the viral load of HastV-MLB was low. Fatal outcomes associated with RVC are rare, and causality in this case remains uncertain due to multiple confounders. No fatal Rotavirus C case has been reported previously. It is important to consider these rare agents in diarrheal outbreaks or unusually severe gastroenteritis, especially when preliminary investigations are unremarkable.
